The clearinghouse handles thousands of inquiries annually from people who are concerned with mental health issues. Clients include mental health care consumers, family members, professionals, and other interested people whom request information and technical assistance about starting and developing self-help projects, self-advocacy projects and consumer-run mental health services. The clearinghouse also provides on-site consultations to individuals and groups interested in self-help group and consumer-run service development. In addition, it sponsors conferences and training events and has developed a wide variety of printed pamphlets and manuals on issues related to developing self-help and self-advocacy projects. A national quarterly newsletter, “The Key”, provides assistance to consumers, their families, advocates, and physicians.

0 Veces vistoSelf-help groups provide an immense amount of service, which mental health professionals do not adequately understand or coordinate with their services. Epidemiological surveys have documented the profiles of self-help users, the amount of self-help use, and the association between self-help use and professional services. The large majority of self-help users use professional services sometimes as a gateway into professional services, other times concurrently with professional service or as aftercare following a course of professional services. The hallmark features of self-help groups: Their use of the experiential perspective, referent power, and reciprocal helping relationships are contrasted with professional mental health services. The essential elements of effective referrals to self-help groups are discussed. At another level, the chapter also discusses the organizational supports necessary for effective collaboration between self-help groups and professional services. While the boundaries between mental health services and self-help groups must be respected, both parties have much to gain by entering into more extensive community partnerships.

Self-help groups are typically framed as therapeutic communities where participants define the agenda creating a space where open and equal interaction can produce individual learning and personal growth. In Norway, however, self-help groups are managed in a way that integrates them in to the health system but insulates them from clinical medicine; an approach that disciplines participants to act in a particular way in relation to the health system.

1 Veces vistoThe concept of “Self-help group” (SHG) has been attracting scientific focus from different disciplines. In general, SHGs can be objectified as a spontaneous civil initiative leading to formation of collective activities through fostering solidarity to tackle any types of life challenges, such as poverty, gender-inequality, discrimination, and any type of marginalization. Also, different groups have different shared objectives; peersupport on medical conditions or both physical and mental disabilities, alcoholic addiction, and environmental issues shared among local community, just to name a few. This research elucidates how different academic disciplines separately analyze SHGs without enough inter-reference and problematizes a fragmentation of SHGs research caused by a lack of interdisciplinary approach, ending in failure of constructive studies. In conclusion, the author reveals a possible framework to grasp the whole picture of SHGs.

0 Veces vistoJürgen Matzat graduated in psychology and since 1977 has worked as a psychological psychotherapist at the Department of Psychosomatics and Psychotherapy at the Justus Liebig University Giessen, Germany, where his research projects include Psychological Therapeutic Self-Help Groups (Prof. M. L. Moeller) and Self-Help Groups for Patients Suffering from Psychological and Psychosomatic Disorders (Prof. M. E. Beutel). He is one of the founders and long-standing board member of a professional organisation, the Deutsche Arbeitsgemeinschaft Selbsthilfegruppen e. V. (German Association of Self-Help Groups) and since 1987 has been head of the contact centre for self-help groups at the Department of Psychosomatics in Giessen. Jürgen Matzat participated in the German government’s two model programmes for supporting self-help groups. He is the editor of the Selbsthilfegruppenjahrbuch (Self-Help Groups Yearbook) and has written a range of publications on self-help. He is also on the editorial board of the International Journal of Self-Help & Self-Care. For many years he has been involved in the healthcare policy surrounding self-help groups. He is the patient representative in the psychotherapy subcommittee of the Gemeinsamer Bundesausschuss (Federal Joint Committee), the psychotherapy admissions and approval committee and the committee for the state of Hesse as well as in other working groups developing medical care guidelines; in addition, he represents the Giessen district in the Gesunde Städte-Netzwerk (Healthy Towns and Cities Network).

This article describes the development and current state of collective self-help in Germany, which is unique in Europe. It has received high recognition from both our society and the state, e. g., in the form of mandatory funding of self-help through the statutory health insurance funds. Since 2004, patient participation in the decision-making bodies of the self-governed German health care system has also been statutorily regulated, first of all in the so called "Joint Federal Committee". The relationship between doctors' organisations, such as the German Medical Association (Bundesärztekammer), the National Association of Statutory Health In-surance Physicians together with the Regional Associations of Statutory Health In-surance Physicians (Kassenärztliche Bundesvereinigung), and the scientific medical societies, has also shifted towards more cooperation, and specific bodies have been set up to this end. Finally, the Agency for Quality in Medicine (AQuMed, ÄZQ) and the Association of Scientific Medical Societies (AWMF) have contributed enormously to the involvement of patient representatives in guideline development groups. The author shares his personal experience in the closing section.
